Membership Forms:

Membership Procedure 2010/2011

1. Please download the Membership and Medical Forms from here (limited paper copies available in the clubhouse).

2. Parents complete and sign the Membership and Medical forms and hand in at the Kit shop with appropriate payment (and photograph as required). Please note that NEW players to the club MUST complete an RFU application, which is available from the Kit shop.

3. 3x Passport-sized photographs are required for ALL new players and those in the U10 and U13 squads. All other squads need to provide 1 passport-sized photograph.

4. Parents to pay Match shirt deposit and collect shirt from the Kit shop.

5. Membership booklet to be issued by the Kit shop.

6. Membership forms (including New player RFU application and photo) to be passed/sent to the Junior Membership Secretary, Sara Tiller.

7. Team Managers can collect player Medical forms from the clubhouse each week. Receipt of this document confirms that the player has been registered and therefore eligible to play in matches.

8. The Junior Membership Secretary will input player information on to RFU Database and provide player Membership cards to each Team Manager.

9. Team Managers are responsible for collecting photographs of existing players in the U10 and U13s squads. The new photos will need to be put onto the players registration card and can be laminated at the kit shop.
